1. I consent to the electronic delivery of documents. The documents subject to this consent include, but are not limited to, the ADV Part II A & B, Privacy Policy, and financial statements.
2. I understand that I will gain access to the SGI web access portal to retrieve the above aforementioned documents, and I confirm that I have such web portal access.
3. I acknowledge most documents posted to the website will be in Portable Document Format (PDF).
4. I acknowledge that I may at any time request and receive at no charge, a paper copy of any specifically requested documents by contacting:

7. I understand and agree that this consent will remain in effect for the entire time I am a client or until I revise and or revoke my consent to electronic delivery.
